Breast Augmentation, also as known as Augmentation Mammoplasty or
Breast Enlarge ment or Breast Enhancement, is the surgical procedure to enlarge the size and the shape of the breasts with breast implants. Breast augmentation is performed for many reasons varying from each patient, some cosmetic and some medical such as  

• patients feel that their breasts are too small
• patients want to correct breast volume after losing weight or after a pregnancy
• patietns would like to correct the difference in size between two breasts
• patients want to improve the shape of breasts that are sagging or have lost firmness, often used with a breast lift procedure

All cases of Breast Augmentation or Augmentation Mammoplasty are operated under general anesthesia in Bangkok-Phuket hospital. The patients are admitted one night in the private room after operation and the patients can be discharged from the hospital in the next day.

The incision has 3 options :
1. Arm pit                             2. Inframammary fold                             3. Areolar incision

I use the incision at the armpit and place the implant under the muscle. In spite of the arm pit incision is more difficult than inframammry fold and areolar incision but I am familiar with this incision and get very good result. No scar on breast at all and the scar at arm pit is excellent (after 6 months). The scar looks like natural axillary-crease.

The operation will be done under endoscopic technique which means that the dissection of the pocket for the silicone implant will be done under vision by the small camera which is better than blind technique and I will see inside the pocket all the time during dissection through the TV monitor, this makes the dissection more accurate and less trauma, less pain, less bleeding and more perfect result.

All cases the implants were placed under the pectoralis muscle. I do not place the implants above the muscle because the long term result is not good especially in the thin patient. This operation has no effect to breast feeding, the patient still can have normal breast feeding after operation.

The implants : I use high cohesive silicone gel implants, texture surface, round implant, texture surface silicone gels implant. I do not use saline filled implant because it has many disadvantages than silicone gel implant. (higher chances to have saline leakeage. Some cases have the sound of water choking inside the implant during jumping, jogging)

Pre-operative care
The patient will be admitted to the hospital on the day of surgery. The patient needs to empty her stomach at least 6-8 hours before operation. If patient uses Aspirin or any anticoagulant drugs regularly, pateints have to stop at least 1 week before operation. Chest x-ray, EKG, routine blood tests will be done on the admission day. If the patients have any allergy history, they have to inform the doctor before surgery.

After one night admission, the pateints will be discharged from the hospital to stay at their hotel or home and come back to the clinic for check up and stitches off at 6 or 7 days after operation.

The patients are allowed for shower since 2 days after operation but are not allowed for swimming both in the pool or sea for 2 weeks until the wound at armpit is completely healed, this is for safety.

The patients will have some pain at the chest muscle and axillary area for 1-2 weeks. Normally they can have light activities after operation 3-4 days, some patients can walk on the beach with bigini suite, some can have a travel trip within Phuket, Krabi or Phang-nga, or shopping in the department store.The elastic bandage will be wrapped around the chest and breast for 2 days after that the patient can use soft bra or sport bra or no bra depending on the patient feeling that which one is the most comfortable and feel less pain.

About 7 days after operation 80% of pain decreased, most of the patients still have a little pain in the second week or third week. Normally after the operation about 3-4 weeks can have normal activities and exercises.

The patients who come from abroad should have time in Phuket at least 5 days for this operation but we recommend to have time about 7 days is better, to make sure that everything is OK before leaving from Phuket.

The limitation for breast augmentation is the breast that is too sagging, a little sagging after breast feeding is still OK for operation but if the sagging is so much with the nipple level is too low is contraindication because the result will not be good, in the case likes this the patient needs breast lift with or without implant augmentation. The problem of breast lift is the scar around the areolar and vertical acar from the areolar to the inframammary fold, the patient who accepts this kind of scar can be performed the breast lift or breast reduction but if the patient can not accept the scar please forget the idea for breast operation.

I quarantee the results of the operation 1 year. If there are any problems that occure from the doctor operating is not good, free of charge for correction for example bleeding, infection, asymmetry that occure from the operation, except in the case that the native breast is asymmetry before operation. The problem of capsular contracture is unpredictable so the quarantee for correction is free only for the doctor fee, the patient has to pay for the hospital cost by herself.

For example, the patient that has heart function abnormality which needs special investigation and medication by cardiologist before operation and during general anesthesia, the patient that has hypo or hyperthyroid that needs extra investigation or medications, the cost for these kinds of problem needs extra payment from the package price.
